These Terms & Conditions (“Terms”) govern purchases from and use of products, impression kits, dental appliances, websites, and related services provided by Vision Dental Lab, located in Idaho Falls, Idaho (“Vision Dental Lab,” “Vision,” “we,” “us,” or “our”).
By placing an order, submitting an impression or scan, accepting these Terms during checkout, or using a Vision Dental Lab product, you acknowledge that you have read, understood, and agree to these Terms.
For products requiring an at-home dental impression, you must also review and accept the separate At-Home Impression Assumption of Risk & Release before proceeding.
1. ABOUT VISION DENTAL LAB
Vision Dental Lab manufactures custom dental appliances for dental professionals and consumers using dental impressions, digital scans, models, prescriptions, and other information supplied to us.
Our role is the fabrication and supply of dental appliances and related products. Except where expressly stated otherwise, Vision Dental Lab does not perform dental examinations, diagnose dental conditions, or replace the services of a licensed dental professional.
2. NOT MEDICAL OR DENTAL ADVICE
Information, instructions, products, impression kits, and services provided by Vision Dental Lab are not a substitute for professional dental examination, diagnosis, treatment, or continuing care.
Vision Dental Lab does not diagnose dental disease or determine whether an individual is medically or dentally suitable for a particular treatment solely through the purchase of a consumer product or submission of an at-home impression.
You should consult a licensed dental professional if you have concerns about the condition of your teeth, gums, bite, restorations, or overall oral health.
Seek professional dental advice before taking an impression or using an appliance if you have:

loose, cracked, damaged, or painful teeth;
-
loose or temporary crowns, bridges, veneers, fillings, implants, or restorations;
-
significant gum disease, bleeding, swelling, or infection;
-
active orthodontic treatment;
-
recent dental procedures;
-
unexplained oral pain or sensitivity; or
-
another condition that may make taking an impression or wearing an appliance unsafe.
3. AT-HOME IMPRESSION KITS
Certain products may require the customer to take a dental impression using an at-home impression kit.
Vision Dental Lab may provide impression trays, base-and-catalyst dental impression material, latex-free nitrile gloves, instructions, and related components.
You are responsible for:

reading all instructions before beginning;
-
following those instructions carefully;
-
accurately disclosing requested dental information;
-
stopping immediately if the process causes significant pain or appears likely to damage a tooth or dental restoration; and
-
consulting a dental professional when you are unsure whether taking an impression is appropriate.
NEVER FORCE AN IMPRESSION TRAY ONTO OR OFF YOUR TEETH.
If an impression becomes unusually difficult or painful to remove, stop and obtain assistance from a qualified dental professional rather than using excessive force.
4. CUSTOMER REPRESENTATIONS
When purchasing a direct-to-consumer custom appliance or using an at-home impression kit, you represent that, to the best of your knowledge:

information you provide to Vision Dental Lab is accurate and complete;

you have disclosed requested known allergies or material sensitivities;
-
you have reviewed the instructions and applicable warnings;
-
you understand that Vision Dental Lab has not performed an in-person dental examination through the at-home impression process;
-
you will not knowingly use the impression kit contrary to its instructions;
-
you will stop if continuing appears unsafe or causes significant pain;
-
you will not knowingly use an appliance contrary to its intended purpose; and
-
you will seek professional dental evaluation when circumstances reasonably warrant it.
Vision Dental Lab may decline, suspend, or cancel fabrication when information or an impression appears insufficient or inappropriate for manufacturing.
5. CUSTOMER-SUPPLIED IMPRESSIONS AND INFORMATION
Custom appliances are fabricated using the impression, scan, model, prescription, selections, and information provided to Vision Dental Lab.
The accuracy of the resulting appliance therefore depends in part upon the accuracy and quality of that information.
Vision Dental Lab may reject an impression that is distorted, incomplete, contaminated, damaged, improperly taken, or otherwise unsuitable for fabrication and may require another impression before manufacturing begins.
Vision Dental Lab is not responsible for problems resulting from materially inaccurate information supplied by the customer or from a customer’s failure to follow impression instructions, except where liability cannot lawfully be excluded.
6. MATERIALS
Vision Dental Lab selects materials appropriate for the particular appliance and intended manufacturing process.
Depending upon the product ordered, materials may include:
Formlabs Dental LT Clear Resin V2
Used in applicable 3D-printed dental appliances. Material is processed according to applicable manufacturer specifications and instructions for its intended application.
Beacon Dental Products Thermoforming Material
Used in applicable thermoformed or suck-down dental appliances.
Dental Impression Material
At-home impression kits may contain a two-component base-and-catalyst dental impression material.
Gloves
At-home impression kits may include latex-free nitrile gloves.
Materials, manufacturing methods, and suppliers may change as technology, availability, regulatory requirements, or manufacturing processes evolve. Vision Dental Lab may substitute an appropriate material suitable for the intended application.
Vision Dental Lab does not represent that every material is universally suitable for every person.
7. MATERIAL SENSITIVITIES AND ALLERGIC REACTIONS
Individual sensitivities and allergic reactions can occur even when a material is used for its intended purpose.
Customers are responsible for informing Vision Dental Lab and/or their dental professional of known material allergies or sensitivities before ordering or using a product.
By using an appliance or impression material, the customer acknowledges the possibility of an individual sensitivity, irritation, intolerance, or allergic response.
To the fullest extent permitted by law, the customer assumes the risk of an individual sensitivity or allergic reaction that was not known to Vision Dental Lab and could not reasonably have been anticipated from information supplied by the customer or applicable manufacturer information.
Immediately discontinue use and seek appropriate medical or dental care if you experience significant swelling, difficulty breathing, severe irritation, or another potentially serious reaction.
8. CUSTOM FIT AND ADJUSTMENTS
Dental anatomy differs from person to person.
Although Vision Dental Lab strives to provide a precise custom fit, a dental appliance may require adjustment.
An appliance should not be forced into place.
Discontinue use and contact Vision Dental Lab and/or a qualified dental professional if an appliance causes:

significant or persistent pain;
-
bleeding;
-
significant gum irritation;
-
unexpected or concerning tooth movement;
-
damage to a tooth or restoration;
-
significant changes in your bite; or
-
another concerning symptom.
9. PROPER USE
Vision Dental Lab products must be used only for their intended purpose and according to applicable instructions.
Unless instructed by Vision Dental Lab or a qualified dental professional, customers should not cut, grind, reshape, heat, repair, modify, or otherwise alter a custom appliance.
Vision Dental Lab is not responsible for damage caused by misuse, unauthorized alteration, improper cleaning or storage, excessive heat, pets, accidental damage, failure to follow instructions, or use contrary to the product's intended purpose, except where liability cannot lawfully be excluded.
10. NO GUARANTEE OF CLINICAL OUTCOME
Individual anatomy, bite, grinding or clenching forces, dental condition, habits, compliance, and other factors vary substantially.
Vision Dental Lab does not guarantee that an appliance will prevent all tooth wear, breakage, grinding, clenching, discomfort, dental disease, or other dental problems.
No statement made by Vision Dental Lab should be interpreted as a guarantee of a particular medical or dental outcome.
11. PRODUCT WEAR AND LIFESPAN
Dental appliances are subject to ordinary wear.
The lifespan of an appliance depends upon numerous factors, including frequency of use, grinding or clenching force, care, cleaning, storage, material, and individual anatomy.
Vision Dental Lab does not guarantee a specific useful lifespan unless a separate written warranty expressly provides otherwise.
12. LOST OR DAMAGED APPLIANCES
Vision Dental Lab is not responsible for appliances that are lost, stolen, misplaced, damaged by pets, exposed to excessive heat, improperly cleaned, improperly stored, intentionally altered, or otherwise damaged after delivery through circumstances outside Vision Dental Lab's control.
Replacement may require additional payment and, depending upon the circumstances, a new impression or scan.
13. PRODUCT VARIATIONS
Because appliances are individually fabricated, minor differences in color, transparency, finish, texture, thickness, dimensions, or appearance may occur.
Such variations do not necessarily constitute a product defect.
14. ORDERS FROM DENTAL PROFESSIONALS
When an appliance is ordered or prescribed by a licensed dental professional, Vision Dental Lab acts as the fabricating laboratory within the scope applicable to that order.
The ordering professional remains responsible for clinical decisions and professional responsibilities applicable to the patient, including examination, diagnosis, prescription, treatment planning, delivery, clinical evaluation, and adjustments where those responsibilities are legally assigned to the professional.
Vision Dental Lab remains responsible for its own fabrication obligations to the extent required by applicable law.
15. WEBSITE INFORMATION
Website descriptions, photographs, educational materials, instructions, FAQs, and other content are provided for general informational purposes.
Website information is not individualized medical or dental advice and should not be treated as a diagnosis.
Vision Dental Lab may update website content as products, processes, materials, or information change.
16. LIMITATION OF LIABILITY
TO THE FULLEST EXTENT PERMITTED BY APPLICABLE LAW, Vision Dental Lab and its owners, employees, contractors, affiliates, and representatives will not be liable for indirect, incidental, special, exemplary, punitive, or consequential damages resulting from the purchase, possession, use, misuse, alteration, loss, or inability to use a Vision Dental Lab product or service.
This provision does not exclude or limit liability that cannot legally be excluded or limited under applicable law.
Nothing in these Terms is intended to waive a consumer's rights that cannot lawfully be waived.
Risks specifically associated with customer-administered at-home dental impressions are addressed separately in the At-Home Impression Assumption of Risk & Release.
17. THIRD-PARTY PRODUCTS AND COMPONENTS
Certain products or components used or distributed by Vision Dental Lab may be manufactured by third parties.
Third-party materials remain subject to applicable manufacturer specifications, instructions, warnings, and limitations.
Vision Dental Lab is not responsible for unauthorized modification or use of a third-party material contrary to applicable manufacturer instructions.
18. COMPLIANCE WITH INSTRUCTIONS
Warnings and instructions supplied with an impression kit or dental appliance form part of the information provided concerning proper use.
Customers should retain and follow those instructions.
Failure to follow applicable warnings or instructions may increase the risk of injury, product damage, poor fit, or an unusable impression.
19. GOVERNING LAW
To the extent permitted by applicable law, these Terms will be governed by and interpreted under the laws of the State of Idaho, without regard to conflict-of-law principles.
Nothing in this provision is intended to restrict a right to pursue a claim in an Idaho tribunal where such restriction would be prohibited by Idaho law.
20. SEVERABILITY
If a court of competent jurisdiction determines that any portion of these Terms is unlawful, invalid, or unenforceable, that provision will be enforced to the maximum extent permitted by law or severed where appropriate, and the remaining provisions will continue in effect to the extent permitted by law.
21. NO WAIVER
Failure by Vision Dental Lab to enforce a provision of these Terms on one occasion does not constitute a waiver of that provision or another provision on a later occasion.
22. CHANGES TO THESE TERMS
Vision Dental Lab may revise these Terms prospectively.
The Terms applicable to a transaction will be the version presented or otherwise made available to the customer in connection with that transaction.
Material changes will not retroactively alter the contractual terms applicable to a completed transaction unless legally permitted and agreed to where required.

ACKNOWLEDGMENT
Vision Dental Lab provides laboratory fabrication services. For direct-to-consumer orders, Vision Dental Lab fabricates the selected appliance from an impression voluntarily taken and submitted by the customer. Vision Dental Lab does not physically take the customer's impression, perform an intraoral examination, diagnose dental conditions, prescribe dental treatment, or perform intraoral fitting or adjustment as part of the direct-to-consumer service.
By affirmatively accepting these Terms during checkout, you acknowledge that you have had an opportunity to review them before completing your purchase.
For products involving an at-home dental impression, acceptance of these Terms does not replace the separate At-Home Impression Assumption of Risk & Release required for those products.
